What it measures

The Yale-Brown Obsessive Compulsive Scale (Y-BOCS) is a clinician-administered measure of obsessive-compulsive symptom severity. The tool has two tabs. The Symptom Checklist records which obsessions and compulsions are present now (past week) or were present in the past, grouped by theme: aggressive, contamination, sexual, hoarding, religious, symmetry, somatic, and miscellaneous obsessions, and cleaning, checking, counting, ordering, hoarding, and miscellaneous compulsions. The Severity Scale rates how much those symptoms affect the patient.

Items and scoring

The ten core items are each rated 0–4 using anchor descriptions. Items 1–5 rate obsessions (time occupied, interference, distress, resistance, control) and items 6–10 rate compulsions on the same five dimensions. Each subtotal ranges 0–20 and the total ranges 0–40. The tool applies these severity bands to the total:

Total (items 1–10)Severity
0–7Subclinical
8–15Mild
16–23Moderate
24–31Severe
32–40Extreme

Supplemental items 1b and 6b (obsession-free and compulsion-free intervals) and investigational items 11–19 (insight, avoidance, indecisiveness, overvalued responsibility, pervasive slowness, pathological doubting, global severity, global improvement, and reliability) can be rated for context. They are not added to the 0–40 total, and checklist selections are descriptive only.

What the report includes

The main report lists the date, both subtotals, the total, the severity band, each core item score, the supplemental and investigational ratings, and the current and past checklist items. Separate buttons copy just the checklist or just the investigational items. Reports are plain text for pasting into an EMR. A blank form can also be printed.

Limitations

Unanswered core items count as 0, so rate all ten items before interpreting the total. Severity bands are a guide and should be read in clinical context. The scale was developed by Goodman and colleagues (Arch Gen Psychiatry, 1989), the reference cited in the tool's report.
