What it measures
The Positive and Negative Syndrome Scale (PANSS; Kay 1987) is a clinician-rated measure of symptom severity in schizophrenia and other psychotic disorders. It requires a trained rater, and ratings reflect the past week. Every item is rated from 1 (absent) through minimal, mild, moderate, moderately severe and severe to 7 (extreme). The tool offers two forms: the brief PANSS-6 and the full PANSS-30.
PANSS-6 and PANSS-30 scoring
The PANSS-6 uses six core items: delusions (P1), conceptual disorganization (P2), hallucinatory behavior (P3), blunted affect (N1), passive/apathetic social withdrawal (N4), and lack of spontaneity and flow of conversation (N6). It reports a positive and a negative subscale (each out of 21) and a total out of 42. Formal mild, moderate or marked cut-offs have not been established for the PANSS-6, so interpret it in context, ideally with a CGI-Severity rating.
The PANSS-30 reports the Positive scale (P1–P7, out of 49), Negative scale (N1–N7, out of 49), General Psychopathology scale (G1–G16, out of 112), a total out of 210, and a composite index (positive minus negative). Severity is shown once all 30 items are rated, using anchors from Leucht and colleagues (2005) that link totals to CGI-S:
| PANSS-30 total | Interpretation |
|---|---|
| Below 58 | Below mildly ill (minimal/borderline) |
| 58–74 | Mildly ill (58 ≈ CGI-S 3) |
| 75–94 | Moderately ill (75 ≈ CGI-S 4) |
| 95–115 | Markedly ill (95 ≈ CGI-S 5) |
| 116–210 | Severely ill (116 ≈ CGI-S 6) |
Marder factor analysis
The full form also calculates the five Marder factors (Marder et al., 1997): positive symptoms (P1, P3, P5, P6, N7, G1, G9, G12; out of 56), negative symptoms (N1, N2, N3, N4, N6, G7, G16; out of 49), disorganized thought (P2, N5, G5, G10, G11, G13, G15; out of 49), uncontrolled hostility/excitement (P4, P7, G8, G14; out of 28), and anxiety/depression (G2, G3, G4, G6; out of 28).
What the report includes
Generate Report copies a plain-text summary. For the PANSS-6 it lists each item rating, both subscales, the total and the interpretation note. For the PANSS-30 it lists every item by subscale, the subscale scores, the total, the composite index, the severity band (or a note that not all 30 items are rated), and the Marder factor scores. The CGI-linked anchors are approximate and should be read in clinical context.
