Why it exists

SMI-FRAIL-20 is a structured checklist for individualized documentation of medical frailty in patients with severe mental illness. It was built in response to the June 2026 CMS interim final rule (CMS-2454-IFC), which narrowed the Medicaid work-requirement medical frailty exemption. As described in the tool, the rule requires states to verify not only a qualifying diagnosis but that the condition significantly impairs the patient's ability to meet the 80-hour-per-month community engagement requirement, and it prohibits relying on diagnosis codes alone. The tool is completed for functioning over the preceding 30–90 days, using chart review, collateral information, and direct interview.

Items and scoring

Twenty items are rated 0 (not present), 1 (mild), 2 (moderate), or 3 (severe), for a total of 0–60. They are grouped into four domains: Symptom Acuity and Safety, Functional Capacity (including ADLs, IADLs, and capacity to sustain 80 hours a month of structured activity), Care Utilization and Treatment, and Support and Systemic Factors. Three items are binary, scored 0 or 3: suicide attempt or self-harm, psychiatric hospitalization, and crisis or ED use, each in the past 12 months.

SMI total (0–60)Band
0–10Minimal
11–25Mild-to-moderate
26–40Moderate-to-severe
41–60Severe

An optional 8-item SUD add-on (0–24) covers withdrawal risk, overdose history, medical complications, cognitive impairment, functional impact, treatment engagement, housing or legal instability, and duration of remission. Its bands are low (0–6), moderate (7–14), and severe (15–24).

What the report includes

The EMR-ready summary opens with the date, the review period, and a generated narrative that names the diagnosis and the most impaired domain. It then gives the clinician's dated behavioral example of functional impairment, the total and band, each domain score, every item response with optional notes, the SUD module if used, and the policy context. If no example was entered, the report inserts a placeholder flagging that one is needed, and every item scored 2–3 should be paired with an example in the chart note.

Limitations

SMI-FRAIL-20 is an educational documentation aid. It is not a validated psychometric instrument, it is not endorsed by CMS or any state Medicaid agency, and it does not determine eligibility; each state sets its own verification standards. The score bands are an organizing framework, not a CMS-published cut-off. The tool notes that a multistate legal challenge to the rule is pending, so confirm current enforcement with your state Medicaid agency.
