However, its bioavailability is poor — estimated at less than 5% under most conditions. The majority of an oral oxide dose remains unabsorbed and exerts an osmotic laxative effect in the colon, which is its primary therapeutic utility (bowel preparation, constipation). For patients seeking systemic repletion — particularly for neurological, psychiatric, cardiovascular, or metabolic indications — magnesium oxide is not the appropriate choice. Despite this, it remains the dominant form in mass-market OTC supplements.
Magnesium Citrate
Magnesium citrate (approximately 16% elemental magnesium by weight) demonstrates meaningfully better bioavailability than oxide, estimated at approximately 25–30% in comparative studies. It is reasonably well tolerated at standard doses, with loose stools occurring if doses are excessive. Magnesium citrate has been used as a bowel preparation agent at high doses and as a general systemic supplement at lower doses. Magnesium Glycinate (Bisglycinate)

Magnesium L-Threonate (Magtein)

Magnesium Malate
Bound to malic acid, magnesium malate has good bioavailability and the additional theoretical benefit of malic acid's role as an intermediate in the Krebs cycle — potentially supporting mitochondrial energy production in conditions characterized by cellular energy deficiency. Small studies have suggested utility in fibromyalgia and chronic fatigue-related muscle pain, likely via combined magnesium repletion and malic acid effects. It is well absorbed and generally well tolerated.
Magnesium Taurate
Magnesium taurate combines magnesium with taurine, an amino sulfonic acid with independent cardiovascular and neurological properties. Taurine modulates calcium channels, reduces blood pressure, and has antiarrhythmic properties — creating a pharmacological rationale for co-delivery with magnesium in cardiovascular contexts. Evidence specifically comparing taurate to other magnesium forms for clinical outcomes is limited; it is primarily distinguished by the potential additive cardiovascular benefit of taurine.
Magnesium Sulfate
Intravenous and intramuscular magnesium sulfate represent the highest-evidence form of magnesium in medicine. IV magnesium sulfate (4 g loading dose, 1–2 g/hour maintenance infusion) is the standard of care for eclampsia seizure prophylaxis and treatment. IV bolus administration (1–2 g over 5–20 minutes) is first-line for torsades de pointes regardless of magnesium level. Migraine Prophylaxis

Insomnia
Abbasi et al. (2012, Journal of the American Geriatrics Society) conducted a double-blind RCT of 500 mg magnesium daily in elderly adults with insomnia and found significant improvements in sleep efficiency, sleep onset latency, serum melatonin levels, and renin levels compared to placebo. The proposed mechanism involves magnesium's regulation of NMDA and GABA-A receptors, and its role in melatonin synthesis. Magnesium glycinate or threonate at 200–400 mg taken at bedtime represents a reasonable recommendation for patients with insomnia, particularly when pharmacological sleep aids are undesirable.
Restless Leg Syndrome
Magnesium 300 mg at bedtime has modest support from small open-label studies for reducing RLS symptoms. Before initiating magnesium for RLS, clinicians should exclude iron deficiency (the most common correctable cause), dopaminergic dysfunction, and medication-related causes (antihistamines, antidopaminergic agents). Magnesium may be a reasonable adjunctive measure but is not established as primary therapy.
Type 2 Diabetes Prevention and Metabolic Health

An important caution: excessive zinc supplementation (above approximately 50 mg/day chronically) competitively inhibits copper absorption, leading to copper deficiency with potential neurological sequelae including myelopathy and peripheral neuropathy.
Boron
Boron is a trace element without an established RDA, though typical supplementation doses are 3–6 mg/day. Evidence suggests boron enhances intestinal magnesium absorption and reduces renal magnesium excretion, effectively improving magnesium retention. Boron also supports bone density (appears to reduce urinary calcium and magnesium loss), modulates estrogen and testosterone metabolism, and has emerging data suggesting cognitive benefits in deficient states. Dietary boron is found primarily in fruits, nuts, and vegetables; deficiency is more common in populations with low fruit and vegetable intake.
Vitamin B6 (Pyridoxine)
Pyridoxine is required for cellular magnesium uptake — B6 deficiency impairs the transport of magnesium into cells even when serum magnesium is adequate. B6 is also an indispensable cofactor in the synthesis of serotonin (from 5-HTP), dopamine (from L-DOPA), and GABA — making its status directly relevant to neuropsychiatric function. B6 deficiency is seen in individuals on isoniazid, oral contraceptives, and some anticonvulsants. Combined Mg + B6 formulations have been studied specifically in ADHD and anxiety. Clinicians must be aware of the established neurotoxicity of high-dose pyridoxine: supplemental doses consistently above 100 mg/day (some reports suggest as low as 50 mg/day with prolonged use) can produce sensory neuropathy. This risk is absent at the 10–25 mg doses common in combined Mg-B6 products, but patients using separate high-dose B6 supplements concurrently should be counseled.
Calcium and magnesium share absorptive transporters in the small intestine and compete at renal tubular reabsorption sites. High calcium supplementation — particularly common in postmenopausal women taking calcium carbonate for bone health — can meaningfully suppress magnesium absorption and reduce renal magnesium conservation. The ideal dietary calcium-to-magnesium ratio is approximately 2:1. In most contemporary Western diets, the ratio is inverted or skewed far toward calcium (estimated 4:1 to 6:1 in high-calcium supplement users), potentially contributing to subclinical magnesium depletion even in patients whose total magnesium intake appears adequate. Clinicians managing patients on long-term calcium supplementation should consider periodic assessment of magnesium status and, where appropriate, concurrent magnesium supplementation.
Phosphorus
High phosphate intake — characteristic of Western diets rich in processed foods, carbonated beverages (phosphoric acid), and protein supplements — can complex with magnesium in the intestinal lumen to form insoluble magnesium phosphate salts, reducing magnesium absorption. Patients with high processed food intake may have impaired magnesium absorption that is not fully corrected by supplementation alone without dietary modification. Additionally, hyperphosphatemia in chronic kidney disease further impairs magnesium balance by altering the tubular handling of divalent cations.
7. Safety, Toxicity, and Dangers
Oral Safety and Tolerable Upper Intake Level
Oral magnesium from food sources has no established toxicity in healthy individuals with normal renal function; excess dietary magnesium is simply not absorbed. For supplemental magnesium, the National Institutes of Health Office of Dietary Supplements establishes a Tolerable Upper Intake Level (UL) of 350 mg/day of supplemental elemental magnesium for adults. This limit is based not on systemic toxicity but on the laxative threshold — doses above this level commonly produce osmotic diarrhea, particularly with poorly absorbed forms (oxide, sulfate). Nausea and abdominal cramping may also occur. Chelated forms (glycinate, threonate, malate) have better GI tolerance and may be used at higher doses with fewer GI adverse effects, though caution is still warranted in renal impairment.
Hypermagnesemia
Hypermagnesemia (serum Mg >1.1 mmol/L) is primarily a risk in patients with renal impairment who take oral magnesium supplements or receive IV magnesium. The kidney is the principal route of magnesium elimination; when GFR is reduced, magnesium accumulates. Clinicians should use oral magnesium supplementation with caution in patients with GFR <30 mL/min/1.73m2, and avoid it or monitor closely in dialysis patients.
The progression of hypermagnesemia symptoms is predictable and follows serum levels:
- Mild (>1.1 mmol/L): Flushing, nausea, headache, fatigue
- Moderate (~2–3 mmol/L): Hypotension, bradycardia, decreased deep tendon reflexes (loss of patellar reflex is the first clinical sign of significant toxicity and should halt IV infusions immediately)
- Severe (~4–5 mmol/L): Respiratory depression, neuromuscular paralysis
- Critical (>6 mmol/L): Cardiac arrest, coma
During IV magnesium infusion (particularly for eclampsia), monitor hourly: patellar deep tendon reflexes, respiratory rate (>12/min required), and urine output (>25 mL/hr required). Loss of the patellar reflex occurs at serum Mg approximately 3.5–5 mmol/L and mandates immediate cessation of the infusion. Calcium gluconate 1 g IV is the antidote for symptomatic hypermagnesemia and should be immediately available at the bedside whenever IV magnesium is infused.
Neonatal Hypermagnesemia
Neonates born to mothers who received IV magnesium sulfate for eclampsia or preterm labor tocolysis may exhibit hypermagnesemia at birth. Clinical findings include hypotonia, respiratory depression, and reduced feeding. Neonates should be monitored for these signs; management is supportive, and calcium gluconate can be administered for severe cases.
Drug Interactions
- Fluoroquinolones and tetracyclines: Magnesium chelates these antibiotics in the GI tract, reducing their absorption by up to 90%. Separate administration by at least 2 hours (magnesium taken 2 hours after or 6 hours before the antibiotic)
- Bisphosphonates (alendronate, risedronate): Same chelation mechanism; take bisphosphonate first thing in the morning on an empty stomach, withhold magnesium for at least 2 hours
- Neuromuscular blocking agents: IV magnesium potentiates both depolarizing (succinylcholine) and non-depolarizing neuromuscular blockers; anesthesia teams should be informed of IV magnesium use preoperatively
- CNS depressants: High-dose magnesium may have additive sedative effects; relevant primarily with IV administration
- Calcium channel blockers: Theoretical additive vasodilatory and hypotensive effects with IV magnesium; monitor blood pressure
- Digoxin: Hypomagnesemia increases digoxin toxicity risk; magnesium repletion is clinically important in digitalis toxicity with concurrent hypomagnesemia
Renal Insufficiency Guidance
GFR 30–60 mL/min: Oral supplementation generally safe at standard doses (200–400 mg elemental Mg/day) with periodic serum monitoring every 3–6 months. GFR <30 mL/min: Reduce dose substantially, avoid high-dose supplementation, and monitor serum magnesium monthly. Dialysis patients: Magnesium is dialyzable; dialysate magnesium concentration should be considered when managing supplementation needs.
8. Practical Prescribing Guidance
Selecting the Formulation
Match the formulation to the indication:
- Neurological/psychiatric indications (anxiety, insomnia, depression adjunct, migraine): Magnesium glycinate 200–400 mg elemental Mg/day; glycinate preferred for cost-effectiveness and dual glycine activity
- Cognitive or specific neuropsychiatric focus: Magnesium L-threonate if the patient can afford it and has tried glycinate without sufficient response; typical commercial dose provides ~144 mg elemental Mg/day
- Cardiovascular indications (hypertension adjunct): Magnesium citrate or taurate
- Gastrointestinal / bowel function: Magnesium oxide or sulfate (laxative effect intentional)
- Fibromyalgia / muscle fatigue: Magnesium malate
- Acute hospital indications: Magnesium sulfate IV/IM exclusively
- General dietary repletion on a budget: Magnesium citrate
When to Check Serum Magnesium
Order serum magnesium when: the patient is on loop or thiazide diuretics, PPIs for >12 months, aminoglycosides, cisplatin, or tacrolimus; when hypokalemia or hypocalcemia is present and refractory to supplementation; in patients with alcoholism, IBD, celiac, or short bowel syndrome; in ICU patients on admission; and whenever the clinical presentation includes unexplained muscle cramps, hyperreflexia, arrhythmias, or treatment-resistant depression/anxiety. Remember that a normal serum level does not exclude intracellular deficiency in high-risk clinical contexts.
When Serum Magnesium Is Insufficient
In patients with compelling clinical deficiency (symptoms, depleting conditions, depleting medications) and a normal serum magnesium, consider ordering RBC magnesium and/or 24-hour urinary magnesium excretion. A low urine magnesium with symptoms suggests dietary/absorptive deficiency; a high urine magnesium suggests renal wasting, which warrants nephrology evaluation to identify the underlying tubular disorder.
When to Refer
- Nephrology: Suspected renal tubular magnesium wasting (refractory hypomagnesemia with elevated urinary magnesium), GFR <30, dialysis patients
- Gastroenterology: Suspected malabsorptive etiology (IBD, celiac disease, short bowel syndrome) contributing to refractory hypomagnesemia
- Endocrinology: Primary aldosteronism; hypoparathyroidism-associated magnesium disturbance
Patient Education Principles
- Start low, go slow: Begin at half the target dose for 1–2 weeks to minimize GI adverse effects, then titrate upward
- Divide doses: Spreading the daily dose into two divided doses (e.g., morning and bedtime) reduces peak GI exposure and may improve total daily absorption
- Take with food: Food slows gastric emptying and may improve absorption and tolerance; exception — bedtime glycinate for sleep can be taken without food
- Separate from antibiotics and bisphosphonates: Counsel patients on the 2-hour rule explicitly
- Duration: In patients with ongoing depleting conditions (chronic diuretic use, IBD, PPIs), supplementation is likely to be long-term maintenance rather than a finite course
- Realistic expectations: For neuropsychiatric indications, effects on mood, sleep, and anxiety typically become apparent within 4–8 weeks of consistent use; migraine prevention requires a 3-month minimum trial
